Job Roles
- Schedule transportation for all PACE participants. Respond appropriately to needs as identified by the interdisciplinary Care Team.
- Coordinate transportation for PACE participants to ensure that the transportation is on schedule, timelines are met, and are at the highest level of operational efficiency.
- Coordinate schedules, cancel schedules, and reschedule trips as needed. Make adjustments to meet unanticipated changes and communicate those changes to the appropriate affected personnel.
- Model and promote service excellence by developing and maintaining positive respectful relationships with PACE participants, families/caregivers, team members, and all staff throughout the organization.
- Maintain all records for transportation, and ensure established policies and procedures are communicated and followed.
- Perform other duties as assigned.
Education/Certifications/Licenses/Registrations
- High School Diploma/GED required.
- Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
Experience/Specialized skills (including Language)
- 1 year experience with transportation scheduling, and organizing routes for pickup and drop off of clients served is required, or equivalent combination of education, transportation scheduling, or transportation industry experience.
- Some experience working with the elderly population is desirable.
- Ability to clearly communicate verbally and in writing.
- Knowledge of the functions and responsibilities of a Driver.
- Ability and skills to effectively plan and organize assignments in the most efficient manner.
- Possess the ability to be flexible, and adaptable in a fast-paced changing environment.
- Bilingual in English/Spanish preferred.

18.00 - 24.00If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
