Description

We are seeking a highly motivated and empathetic Social Worker to specialize in supporting Veterans, Native Americans, and individuals recently released from incarceration. As a Social Worker, you will play a critical role in providing individualized assistance, counseling, and advocacy to help these populations navigate challenges and access necessary resources. Your role will involve collaborating with a multidisciplinary team, building relationships with community organizations, and promoting the rights and well-being of your clients.

Responsibilities:

· Conduct comprehensive assessments to identify the unique needs, strengths, and goals of the underserved including, Veterans, Native Americans, and individuals recently released from incarceration

· Develop and implement individualized service plans in collaboration with clients, their families, and relevant stakeholders

· Provide counseling, support, and crisis intervention to address mental health, substance abuse, and trauma-related issues

· Assist clients in accessing community resources, such as healthcare, housing, employment, education, substance abuse treatment, mental health services, and legal assistance

· Advocate for clients' rights and ensure their voices are heard in decision-making processes

· Collaborate with a multidisciplinary team, including case managers, healthcare professionals, legal advisors, and community agencies, to provide integrated and coordinated care

· Build relationships with local organizations, government agencies, and community partners to enhance the resources and support available to clients

· Maintain accurate and up-to-date client records, adhering to confidentiality and regulatory requirements

· Stay informed about relevant policies, procedures, and best practices related to serving Veterans, Native Americans, and individuals transitioning from incarceration

· Participate in regular team meetings, case conferences, and professional development

Qualifications:

· Master's degree in social work, counseling or a related field from an accredited institution

· Valid associate license or eligibility for licensure (CSWA, LMSW or QMHP preferred)

· Previous experience in counseling, social work or a related field, preferably working with Veterans, Native Americans, or individuals transitioning from incarceration

· Strong knowledge of the unique challenges faced by these populations and familiarity with available resources and support systems

· Excellent assessment, counseling, and crisis intervention skills

· Culturally sensitive and aware of the historical and social factors affecting Veterans, Native Americans, and individuals with incarceration experience

· Effective communication and interpersonal skills, with the ability to establish trust and build rapport with clients and stakeholders

· Ability to work independently and as part of a team, managing multiple priorities and meeting deadlines

· Proficiency in relevant computer applications and record-keeping systems

· Valid driver's license and access to reliable transportation (if required for the position).

· Addresses of all residences

· Personal and Professional References

· Names and addresses of family members

· Arrests and criminal history

· Traffic driving record

· Military service

· Employment history

· Drug use

· Education history

· Memberships in professional organizations

· Financial information

Copies of the following documents:

· High school diploma/GED certificate

· Social Security card

· Driver’s License

· Birth Certificate (Certified copy);

· Any other documents, certificates, resumes, performance evaluation reports, etc., which show your abilities and training for the position

3. Additional selection steps - include a background investigation.

· Employment is contingent upon successful completion of comprehensive background investigation

Pay: $28.00 - $35.00 per hour
